Which size should I order?

Go by who is under it rather than by your mattress. Twin (59" x 79") suits one adult. Queen (71" x 87") is the right call for two. King (79" x 91") gives two adults extra overhang on each side, which matters if one of you pulls.

Is it warm enough for anything but summer?

On its own it is a summer and shoulder-season quilt, and in an air-conditioned bedroom it works year round. In a cold winter room most people layer it under or over a heavier duvet rather than using it alone.

Does it need a duvet cover?

No. It is a finished quilt with a bound edge and a contrast reverse, so it is designed to be used as-is and washed as-is. You can put it in a cover if you prefer, but it adds a layer that works against the cooling.

How do I wash it?

Machine wash cold on a gentle cycle and tumble dry low, or line dry. The wave quilting is what stops the fill migrating in the wash, so it comes out the same shape it went in.

Will it actually make a difference if I sleep hot?

It is a lighter, more breathable layer than a standard duvet, so less heat is held against you. It is bedding, not a cooling device — it won't lower the temperature of your room, and if your mattress is the thing storing heat you may want a cooling pad underneath as well.
